Full Time Staff

Medical and Dental Coverage

New employees are qualified to enroll in our insurance programs effective the first day of the next month following a SIXTY DAY waiting period from the date of employment.   

Medical coverage is provided by CareFirst BlueChoice, a managed care organization. There are two medical plans available: Blue Choice HMO or Blue Choice HMO HSA. 

The HMO plan requires the employee to select a Primary Care Physician from the provider directory. Referrals to specialists are required under this plan. Primary care services require a $30.00 co-pay and specialty care services a $40.00 co-pay. A formulary based prescription drug benefit is provided after a $250 deductible per person is met with a three-tier ($15/25/50) co-pay structure.

The HMO HSA plan requires the employee to select a Primary Care Physician from the provider directory but does not require referrals to see specialists. This plan has an annual deductible of $1200 per individual or $2400 per family for in network benefits and has an HSA feature. If you choose this plan, then Pathways will fund the amount of the annual deductible into an HSA account in your name. Primary care services require a $20.00 co-pay and specialty care services a $30.00 co-pay. A formulary based prescription drug benefit is provided after the annual medical deductible per person is met with a three-tier ($0/25/45) co-pay structure.

Dental coverage is  also provided by CareFirst BlueChoice.

Pathways currently pays 70% of the premium for the dental plan and HMO options only. This percentage is subject to change. Employees selecting either of the point of service options are responsible for the full cost of these enhancements.

Life and Disability Insurance

Life, accidental death and dismemberment, and short and long term disability insurances are provided by Mutual of Omaha Insurance Company and United of Omaha Life Insurance Company.

Pathways currently pays 70% of the premium for life, accidental death and dismemberment, and short term disability insurance. The employee pays 100% of the premium for long term disability insurance. These percentages are subject to change.

Other supplemental insurance plans are available through AFLAC.

Pension

The employee is eligible immediately upon employment to enroll in our tax deferred annuity program. After one plan year, employees are eligible for the employer match and the discretionary contribution if they have worked 1,000 hours or more.

Leave Accrual

For full-time staff, leave will accrue per pay period according to the following schedule:

Full Time Staff – Sick Leave
  0-6 months: 3.4 hours
  6 months - 2 years: 3.9 hours
  2 years or more: 5.6 hours

Full Time Staff – Annual Leave
  0-6 months: 3.4 hours
  6 months - 2 years: 3.9 hours
  2 years or more: 5.6 hours

For part-time staff, leave will accrue per pay period according to the following schedule for part-time employees who are working a regularly scheduled 40- 70 hours per pay period.

Part Time Staff – Sick Leave
  0-6 months: 3.4 hours
  6 months - 2 years: 3.9 hours
  2 years or more: 5.6 hours

Part Time Staff – Annual Leave
  0-1 year: 1.3 hours
  2-4 years: 2.2 hours
  More than 4 years: 3.3 hours

For part-time staff, leave will accrue per pay period according to the following schedule for part-time employees who are working on a regularly scheduled basis less than 40 hours per pay period:

Part Time Staff – Annual Leave – 1.3 hours
Part Time Staff – Sick Leave – 1.3 hours

 Substitute staff are not eligible to accrue leave.

* Personal leave is granted to eligible employees every year on January 1, and must be used by December 31. 
   Personal leave is not accrued and does not carry over to the next year. 

Pathways offices close in observation of the following holidays:

    • Independence Day
    • Thanksgiving Day
    • Christmas Day
